This is the way medicine is meant to be.What you'll do: - Conduct outreach to patients from diverse populations to engage them in population health initiatives
- Build rapport and establish trusting relationships with patients to promote their active participation in care
- Identify barriers to healthcare access and address social determinants of health that impact patient well-being
- Educate patients on available resources, preventive measures, and self-care strategies
- Perform comprehensive medical and behavioral assessments to evaluate patients' physical, psychological, and social needs
- Identify and address mental health issues, substance abuse, domestic violence, and other psychosocial factors
- Collaborate with healthcare providers to develop individualized care plans based on assessment findings
- Advocate for patients' needs and preferences in care planning and decision-making processes
- Collaborate closely with a multi-disciplinary team, including physicians, nurses, care coordinators, and community partners
- Participate in care conferences and team meetings to discuss patient cases and develop integrated care plans
- Share expertise on social determinants of health and provide guidance to the team on addressing patients' social and emotional needs
- Coordinate referrals to community resources, mental health services, and support groups
- Maintain accurate and up-to-date documentation of patient interactions, assessments, and care plans
- Adhere to privacy regulations and ensure the confidentiality of patient information
- Provide timely and thorough reports to the multi-disciplinary team regarding patient progress and interventions
