POSITION SUMMARYRN Case ManagerPOSITION DUTIESCollaborates with the medical provider and all interdisciplinary team members to establish individualized medical goals that maximize patient healthcare outcomes.Patient needs should be addressed in a prompt, efficient manner. Works closely with providers and other clinical staff to facilitate and coordinate patient referrals to Surgery/Specialty Clinic. Reviews referral records to ensure all appropriate information is available for providers review.
