Description:We are seeking a highly motivated and attentive Community Resource Liaison (BSW) to join our team at PACE of Southwest Michigan! The Community Resource Liaison is responsible for providing social work case management services to PACE participants and performs duties under the direction of the Social Work Supervisor (SWS). If you enjoy working with the senior population, finding solutions and resources, and providing excellent service, we encourage you to apply.Requirements:Education: Bachelor's degree in Social Work or other related field required.Experience: 1 year experience in a health-related area preferred.Number/Type of Employees Supervised: NoneLicensure, Registry, or Certification Required: Licensed BSW in the State of Michigan preferred, valid Driver's License with a vehicle and safe driving record required.Special Training: Only acts within scope of practice. Meet a standardized set of orientation and/or individual competencies for the specific position description established by PACE of Southwest Michigan and approved by CMS before working independently.Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.Specific Requirements and Considerations: May be subject to exposure and handling of infectious waste, diseases, conditions or hazardous chemicals, etc., including potential exposure to TB, AIDS, and/or Hepatitis B.Ages of Patients Rendered Care: Adult, GeriatricKey Responsibilities---Communicates participant changes to the Interdisciplinary team (IDT) and documents in the participant's medical record.---Refers participants and families to appropriate community agencies, acts as liaison with such agencies and as advocate for participants.---Ensures completion of the Care plan review process and associated documentation.---Assists with respite or permanent placements; prepares and sends required referral paperwork and coordinates admissions.---Ensures participants maintain Medicaid eligibility through completion of Medicaid Applications & Redeterminations.---Receive and return calls on behalf of the social work department.---Prepares correspondence, forms, reports, and other documents on behalf of the social work department.---Conduct participant screening for depression, anxiety, and cognition.---Conducts status checks for participants by phone.---Participates in program and policy development as needed.---Actively participates in the quality improvement plan.---Serves on internal and external committees when appropriate.---Abides by all established PACE of Southwest Michigan policies.---All other duties as assigned within job scope.Skills and Knowledge---A high level of autonomy and comfortability in decision-making.---Knowledge of physical, mental, and social needs of frail, older adults.---Advanced ability to prioritize tasks and strong organizational skills.---Ability to work within the Interdisciplinary Team (IDT) setting with a focus on collaboration and teamwork.---Superior verbal and written communication skills.---Excellent attention to detail.---Proficient in Microsoft Outlook, Excel, and Word---Ability to work a flexible work schedule.---Comfortability utilizing a computer and/or laptop, printer, and phone.---Ability to utilize the organization's electronic medical record system.Physical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
