Key Responsibilities

Conduct clinical evaluations and assessments to determine case management eligibility and individual needs
Develop and implement case management plans in collaboration with members and healthcare professionals
Maintain a member caseload for regular outreach, support, and documentation of interventions and goal achievements



Required Qualifications

Graduation from an accredited Licensed Vocational Nurse program
Current unrestricted LVN/LPN compact license
2+ years of recent clinical experience as an LVN/LPN
1+ years of care management, disease management, or discharge planning experience
