Job Details - Candidates with an AZ home address on their drivers license will not be considered - please include a copy in submission
- Banner has a specific cover page! PLEASE vet all locations to the travelers and add all experienced departments candidates are willing to ACCEPT/work in.
- Start Date should be on or very quickly after the Order Start Date listed.
- All RTO should be listed on the cover page, must be less than 7 days.
- RTO will not be approved in the first week of assignment or holidays.
- All facilities the traveler is open to working in should be listed on the submission.
- If interested in 'any' location, please include what states (I.E. Any AZ, Any CO, Any state, etc.)
- All shifts the traveler is open to working on should be listed on the submission.
- All departments the traveler is open to working must be checked on the cover page.
- All candidates submitted should be preclosed and ready to accept.
Job Requirements - 24 months minimum of current acute CT Tech experience - REQUIRED
- Certifications must be American Heart Association OR American Red Cross, active upon submission and INCLUDED IN SUBMISSION - REQUIRED
- No License Hits - REQUIRED
- Active License (verification must be less than 30 days old)
- Skills Checklist specific to the discipline (must be less than a year old)
- Flu vaccination - Banner declinations can be applied for (reach out for forms)
- Driver's License
Schedule Information - Banner will not approve RTO within the first week or holidays. Can not exceed 7 days.
Unit Specific Information - Banner Cover Page completely filled out
- Resume with all facilities, facility size, beds, trauma level, and units listed
