Key Responsibilities

Develops and assesses ongoing care plans/service plans and collaborates with providers to address member needs
Coordinates care between members, caregivers, and care provider teams to ensure appropriate services
Monitors and revises care plans based on member status and collaborates with stakeholders to meet care goals


Required Qualifications

Active NY State RN license
Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
4 - 6 years of related experience
Knowledge of state and federal regulations regarding healthcare
