Key Responsibilities

Conduct high volume outbound calls to assist members and evaluate health status
Answer inbound calls and assist members with their inquiries
Identify patients with missing measures and work to close these gaps through education and appointment setting


Required Qualifications

Current, unrestricted LVN/LPN license
3+ years of clinical experience as a LVN or LPN
3+ years of experience in the health industry or a large complex organization
