Key Responsibilities:

Perform medication reviews to identify and resolve drug therapy problems
Collaborate with the care team to develop and implement patient-specific care plans
Conduct medication reconciliation post-discharge for high-risk patients


Required Qualifications:

Doctor of Pharmacy (PharmD) degree or 10+ years of clinical experience
Active, unrestricted Pharmacist License in Utah
Post Graduate Training (Pharmacy Residency or Fellowship) or 3+ years of equivalent clinical pharmacy experience
