Key Responsibilities

Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
Develop and monitor ongoing care plans in collaboration with healthcare providers and community resources
Provide education to members and families on healthcare processes and available resources


Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
2 - 4 years of related experience in healthcare
RN - Registered Nurse - State Licensure and/or Compact State Licensure required
Bilingual Spanish speaking is preferred
