Key Responsibilities:

Deliver coordinated, patient-centered virtual care management via telephone and video
Generate impactful care plans with members and the multidisciplinary care team
Assist members in navigating complex medical conditions and the healthcare system


Required Qualifications:

Bachelor of Science in Nursing (BSN)
Must reside in a compact NLC state with an active Compact RN license
2+ years of experience in a Newborn/Pediatric setting
Current CCM Certification
Willingness to work Monday-Friday, 9am-6pm PST More...
