Key Responsibilities

Evaluate individual member needs and develop personalized care plans
Collaborate with healthcare providers, insurers, and other stakeholders for seamless care delivery
Track member progress and adjust care plans to optimize outcomes


Qualifications & Requirements

Active Compact State RN License
At least 2 years of current/recent Case Management experience in a Health Plan
Special Needs Plans (SNP) experience is highly preferred
Strong computer experience, particularly with Microsoft Office
