Key Responsibilities:

Collaborate with other healthcare professionals to develop and implement patient care plans
Follow up with patients to monitor health metrics and provide guidance on lifestyle changes

Required Qualifications:

Active, unrestricted RN license in a state within the Nurse Licensure Compact
