Job Details - Works under the direct supervision of the Director and Clinical Pharmacy Coordinator to assist in optimizing patient's drug regimen.
- Maintains appropriate records for medications and other pharmaceutical supplies ordered by physicians, dentists, and other qualified prescribers.
- Work is accomplished in accordance with the policies and regulations of local, state, and federal authorities, as well as accepted standards of practice.
- Performs dispensing and other duties as assigned.
- Should be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her unit.
- Performs drug regimen review to evaluate and assess the patient's medications for related problems.
- Identifies therapeutic duplications; reviews drugs and allergies; reviews for drug-drug, drug-nutrient, or drug-laboratory test interactions; evaluates if doses are appropriate for age, renal and liver functions; assists with patient/family education when necessary; monitors patients on clinical drug protocols; attends codes; attends rounds and patient care conferences as needed; assists with pain management as needed; provides drug in-services to nursing, pharmacy, medical staff, and other ancillary personnel as needed.
- Identifies the presence of, or potential adverse drug events. Reports as necessary.
