The Role
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide PRN care for an extended care patient with trach, vent, and G-tube needs. You will be working in the patient's home, ensuring their safety, comfort, and medical stability as needed.What You'll Be Doing

• Delivering specialized care for a patient with trach, vent, and G-tube
• Monitoring the patient's respiratory and nutritional health during your shifts
• ?Administering ?medications and tube feedings
• Performing routine assessments, interventions, and adjustments as necessary
• Communicating with the care team and maintaining accurate patient records
• Developing strong, supportive relationships with the patient and their familyWhat Makes You a Great Fit
  • Current Colorado LPN or RN license
  • Experience with trach, vent, and G-tube care preferred
  • Current BLS/CPR certification
  • Strong clinical and communication skills
  • Ability to work independently in a home care setting
