- Provide home-based physical therapy to pediatricpatients who have lost mobility, strength and functionality due to an illness or injury
- Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patients plan of care and goals
- Observe movements, conduct assessments, document progress and modify care plan as needed
- Assist pediatric patient with exercises and hands-on therapy to manage pain and increase mobility
- Educate pediatricpatients on their plan of care, established goals, exercises and self-care
