- Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury
- Work as part of a home health team, which may include an RN, LPN, CNA, HHA, OT, and SLP, focused on the patients plan of care and goals
- Act at the direction and under the supervision of the treating Physical Therapist and in accordance with the plan of care
- Assist the treating Physical Therapist in documenting exercises in the patients chart and printing out exercises for home use as directed
