Collaborates with members of health care team to assist patients in achieving established outcomes of care. Reports to Nurse Manager. - - The LPN is responsible for the collection of health information and communicating pertinent information to the physician and the health care team. - The LPN assists in carrying out the physician's plan of care, provides patients with health information and education and assists in coordinating care and community resources required by the patient.
