• Provide professional nursing services under the direction of a RN
• Apply nursing skills to facilitate problem resolution and achieve patient goals
• Maintain high standards of professional conduct in relation to confidential information
• Prepare, submit and maintain required documentation Required Experience/Skills:
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
  • Current nursing license in the practicing state
  • Valid driver's license, auto insurance, and reliable transportation
  • Current CPR certification
  • HCHB (Home Care Home Base) EMR experience a Plus
