Job Purpose: As a HOPE Team Outreach Specialist at Aspire Health Partners, your job purpose will be to provide a multitude of essential functions regarding reintegration into the community of people who are homeless. Job Functions - Providing outreach services
- Conducting assessments and completing referrals
- Assisting clients with obtaining necessary documents to obtain housing
- Negotiating the criminal justice system and advocating for diversion when appropriate
- Adhering to contractual obligations
- Serving as acting representative to committees and community planning
- Completing required trainings and participating in required meetings
- Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures.
Position qualifications: Ability to receive and maintain a(n) - Level II Background clearance
- Acceptable Motor Vehicle Registration driver's license record in accordance with the underwriting guidelines set by Aspire insurance company as described in the AHP Corporate Transportation Policy 3.9.1
- High School Diploma is required. Bachelor's Degree is preferred.
- 1 to 2 years of experience working with targeted population may substitute for higher education.
