Job Description & RequirementsRN Case ManagerStartDate: 2/10/2025 Pay Rate: $2000.00 - $2700.00POSITION SUMMARY - RN Case ManagerPOSITION DUTIES - Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Floating among sister campuses requiredMINIMUM REQUIRED QUALIFICATIONS - FL/compact RN license; BLS; Accredited nursing degree; 2+ years acute hospital case management experienceLENGTH OF ASSIGNMENT - 13 weeksSHIFT / HOURS PER WEEK - . Weekend rotation of one weekend day every 3 weeks.
