Responsibilities

Acts as a liaison between hospice and the facility to maintain effective communicationUpholds a positive image to the public through effective planning, programming, advertising, special events, and care managementSpecializes in professional Nursing care to patients in facilities Interacts with patients, families, physicians, and other Health team membersReviews admission data as defined by policies and proceduresAssesses patient status and responds in timely mannerImplements priorities for Nursing action in logical sequence according to patient's needsFormulates a plan of care by the appropriate selection of Nurse Practice standardsSupervises the provision of services provided by LPNs based on the plan of care and provides feedback on performance as indicated













Qualifications










• Associate degree in Nursing with current license
