Key Responsibilities:

Assess health status and establish goals to meet identified healthcare needs
Plan, implement, and evaluate responses to the plan of care, including post-discharge assessments
Collaborate with multidisciplinary teams and provide education on health maintenance and disease prevention


Required Qualifications:

Current unrestricted RN licensure in the state of residence
Compact multistate RN license
2+ years of experience as an RN
1+ years of Case Management experience
