Key Responsibilities

Engage members and their support systems through assessments and care planning
Facilitate inter-disciplinary care team meetings and coordinate care for members
Educate members on treatment options and community resources to support informed decision-making


Required Qualifications, Training, and Education

Nursing degree from an accredited program or Bachelor's degree in a health care field
A minimum of three years of experience in nursing, social work, or a related healthcare profession
Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor
Advanced degree associated with clinical licensure is preferred
Three years of Medicaid and/or Medicare managed care experience is preferred More...
