Key Responsibilities

Collaborate with a multidisciplinary care team to provide member-centered care
Facilitate orientation groups and assist members in achieving their care plan goals
Advocate for members and connect them with community resources and support services


Required Qualifications

Associate's degree or higher in social or human services preferred
Two years of experience in case management, care navigation, or peer support required
One year of experience in a social service or behavioral health setting preferred
Ability to work with diverse populations and maintain professional boundaries
