Key Responsibilities

Lead and expand the Population Health Management Program (SPHMP) to meet financial and clinical goals
Ensure compliance with contractual obligations and deliverables for health care plan partners
Analyze team performance and implement continuous improvement plans to enhance member outcomes


Required Qualifications

Qualified Mental Health Professional (QMHP) with relevant clinical licenses (LCSW, LPC, LMFT, PhD, or PsyD)
10+ years of experience in population health management
Experience in leading and implementing successful programs nationally
Bachelor's degree in behavioral health, social work, or health care management required
