    Job Requirements
    • RN Medical/Surgical specialty
    • Vaccines or Positive IGG Titer required
    • Fit Test required annually for all disciplines
    • Documentation of Hepatitis B vaccination series completion or declination documentation required
    • Influenza Vaccine required annually, starting September 4th, 2023
    • Proof of immunity or vaccination required for Varicella
    • Tdap Vaccine valid for 10 years from administered date required
    • Pre-assignment physical exam documentation required, updated annually
    • TB testing (2 Step PPD or IGRA) and proof of immunity required
    • Professionals working on a compact license must have address on Driver's license matching the home state for the compact license
      Unit Specific Information
      • Medical/Surgical unit expertise required
      • Ascension Medical Surgical Exam required
      • Ascension Pharmacology Exam required
      • Ascension Skills Checklist completion required annually
        Additional Details
        • Not accepting exemptions for vaccines in specific areas
        • Chest X-Ray required for positive TB record
