  Job Requirements
  • Must have active Nursing licensure in the state of Indiana
  • Complete facility-specific pharmacology exams
  • Complete facility-specific medical surgical exams
  • Flu vaccine required annually
  • MMR, Hepatitis B, Varicella, and Tdap vaccinations required or proof of immunity
  • Fit Test done annually
  • 2 Step PPD or IGRA and other TB testing measures as required
  • Proof of a negative chest X-ray if PPD is positive
  • Must complete the Ascension Skills Checklist annually in specialty
