Specialized Case Manager - Redford Michigan

The Case Manager provides community based support and advocacy to adults with mental illness who may be at risk of psychiatric hospitalization, lacking sufficient support and currently involved with the judicial system. This professional collaborates with community and state hospitals and works on a multidisciplinary team in treatment planning and interventions. Essential Duties: - Completes all required trainings
- Reports to work on time as scheduled and at the location assigned
- Convey accurate progress and monitoring reports to the local and or State Court systems through competent court report compilation and verbal testimony.
- Adhere to all court policies pertaining to adjudicated clients assigned to the agency including compliance with court ordered requirements
- Document electronically accurate and timely progress notes regarding client progress through contacts with clients, providers, family and other members of the treatment team.
- Coordinates with members of the treatment team including therapists, identified family members, clients, and others as determined appropriate.
- Attend and participate in Case Management in-services and staff development trainings.
- Complete progress and chart notes for all outreach and monitoring occurrences within 24 hours.
- Completes or coordinates and submits all required court documentation including petitions, clinical certs, 90-day reports, 6-month reviews, clinical assessments, IPOS and IPOS reviews within required timeframes.
- Provides face-to-face clinical services and monitors individual at the required frequency as identified in court orders (AOT, NGRI) and individual plans of services.
- Coordinates with local and state psychiatric hospitals to improve treatment engagement, participate in discharge planning meetings and reduce hospital recidivism. QUALIFICATIONS:
- At least two years of experience working with at-risk populations preferred.
- Previous court-related mental health experience preferred.
- Knowledge of and experience with screening and assessment tools.
- Experience with industry standard e-mail, Microsoft applications and Electronic Heath Record.
- Strong organizational and time management skills, flexibility, strong work ethic, ability to work independently and meet deadlines.
- Skills and ability to implement crisis interventions as necessary.CORE COMPETENCIES:

(BSW will be considered with five years of experience in mental health)
