Home Health Nurse

Responsibilities:

• Deliver comprehensive patient assessments, wound care, medication administration, and other skilled services in the home setting.
  
• Provide patient education to patients and families for a wide range of diagnoses, empowering them to effectively manage their conditions at home.
  
• Facilitate communication among all members of the patient's care team, including family members, therapists, social workers, home health aides, and physicians.
  
• Complete accurate and timely documentation of services provided.

    Requirements:
    
    
    
    • Registered Nurse with active, unrestricted license in the State of Michigan.
      
    • At least one year of clinical experience as a Registered Nurse.
      
    • Current CPR Certification.
      
    • Reliable transportation with a valid state driver's license and current car insurance.
      
      Preferred Skills:
      
      
      
      • One year of Home Health experience.
        
      • Community health and a wide range of Medical-Surgical experience.
