This RN role will serve the southwest territory of our West Michigan home care service area, traveling to patient homes between Grand Rapids and Gun Lake.Under the direct supervision of the Nursing Manager, and in collaboration with the interdisciplinary team, the Registered Nurse uses clinical judgment skills to access and implement the treatment plan for patients. The RN assesses monitors, detects, plans and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatment.Communicates effectively with staff, residents and families. Assesses, develops, and maintains an individualized patient plan of care with the patient.Performs Medication Administration Pass according to regulatory guidelines. Informs MDS coordinator of significant resident/patient changes needed for completion of the MDS.Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Identify problems and emergency situations, and initiate immediate "life saving" measures in the absence of a physician.Teaches and supervises the care provided by CNA, LPN or other nursing service personnel according to regulatory and agency guidelines.Implements the plan of care by providing nursing services, including teaching, treatments, diagnostics, and preventive procedures, regularly evaluating the patient's progress toward established goals recommending revisions to the plan as necessary, and planning for termination of service or discharge. Assumes a leadership role and delegates appropriately to support staff to facilitate safe, quality patient care in a cost-effective manner.Participates in ongoing process improvement activities through the application of clinical paths, QA processes and other related activities.
