Vista Prairie at Ridgeway is seeking a Community Care Coordinator to join our compassionate team. As a dedicated Community Care Coordinator, you will be responsible for creating appropriate staff schedules while also maintaining resident care levels. You will embrace a resident-centered approach, treating every individual with unwavering dignity, regardless of their disease, diagnosis, or progression. While providing a therapeutic environment that maintains each resident's highest level of physical, social, and physiological well-being, you will find ways to bring smiles to the faces of those you serve. You will also answer resident emergency calls promptly, assist in transporting residents to/from appointments, activities, meals, and social programs as necessary. As a Community Care Coordinator, you will also assist residents in preparing for activity and social programs as necessary and much more! Together, we can make a meaningful impact on the lives of our residents. Schedule: Full-Time Evening Shift 2PM - 10PM. Monday-Friday with some flexing as needed. Off-hour scheduling coverage every 3 weeks.What you will need: - High school diploma or GED required.
- Must be 18 years of age or older.
- Experience as a CNA or Resident Assistant in SNF or Assisted Living required.
- Staff scheduling experience a plus.
