We are hiring for a Home Care Nurse (LPN/RN) to join our nursing team to support our clients in their homes. We offer world class home care with small community values. We are looking for Full-time Registered Nurse or Licensed Practical Nurse work with a 16-year-old male in his home. We welcome new grads and will train! We have several clients in the Twin Cities area.

- Case is located Rosemount, MN
- 16 yr. old Male
- CP
- Von Willebrand's Disease
- Developmentally Delayed
- Trach
- Vent/BiPap at Night
- NOC shifts only 9pm - 7am
- E/O Weekend Shifts (Fr, Sa, Su) 11am - 9pm

Responsibilities:
- Administer nursing care to client
- Help maintain patients' treatment plan
- Monitor and report changes in patient symptoms and/or behaviors
- Maintain accurate patient medical records
- Provide advice and support to patients and their family members

Qualifications:
- Work from a place of caring and honoring the dignity of all clients
- RN or LPN Minnesota License
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills

Salary:
