    • Active MS or compact State Registered Nurse License
      
    • Valid BLS
      
    • Must be RN Case manager with at least 2 years Acute Care Management experience in discharge planning experience. -
      
    • Discharge Planner with Level 1 and EPIC expr REQUIRED
      
    • Graduate of an Accredited School of Nursing
      
    • Additional certifications may be required
