Case Management Registered Nurse (RN): A Case Management Registered Nurse is a nurse who specializes in coordinating and managing patient care to ensure that patients receive the right care, at the right time, and in the right setting. They work in hospitals, insurance companies, and healthcare organizations, among other settings. Responsibilities may include: Assessing patients' health status and medical history Developing and implementing care plans Coordinating with interdisciplinary teams to ensure that patients receive appropriate care Monitoring and evaluating patient progress Assisting patients and families in navigating the healthcare system Ensuring that patients receive appropriate care in a cost-effective manner Documenting patient care and outcomes.
