If you are a Social Worker with strong clinical experience/skills and you love to care for patients, come and join our incredible team!About the Job:The Elliot position supports the Solinsky Center for Cancer Care in Manchester and Londonderry, including Medical Oncology, Radiation Oncology and Breast Health Services, providing ad-hoc support services and support groups for patients.What You'll Do:

• Collaborates and participates with the primary nurse and the multidisciplinary team in the ongoing clinical social work assessment of each patient
• Identifies relevant community resources and links them to patients as appropriate
• Provides supportive counseling and crisis intervention for patient/family as needed
• Coordinates with key personnel at all community health facilities, which treat oncology patients, to ensure patient needs are met
• Coordinate's homecare referrals, acute rehab, and nursing home referrals as defined
• Serves, as a resource/liaison to community social service agencies relative to identified patient/family needsWho You Are:
  • Master of Science in Social Work, Required
  • One (1) to three (3) years of experience in working with Oncology patients and cancer survivors, Preferred
