Key responsibilities include:Assessments & Care Plans: Evaluate patient needs and develop/adjust care plans.Documentation: Complete timely assessments and ensure accurate records.Education: Provide patient/family education and document interventions.Case Management: Prioritize care needs and collaborate with the team.Safety & Compliance: Follow safety protocols and manage medications.Team Support: Participate in meetings and mentor new staff.Qualifications: RN or LCSW with hospice experience preferred.
