Duties and Responsibility
--- Assessment and Treatment: Conduct thorough assessments of patient wounds, which maybe chronic or acute and develop individualized treatment plans.
--- Implement advanced wound care techniques, including debridement, dressing selection, skin grafts and infection control measures.
--- Collaboration with Healthcare Teams: Work closely with other healthcare professionals at each facility to coordinate care and provide updates on patient progress to ensure continuity of care.
--- Manage a travel schedule to visit multiple skilled nursing facilities on a regular basis.
--- Ensure timely and efficient care delivery.
--- Maintain accurate and comprehensive patient records, documenting all assessments, treatments, and patient/family interactions.
--- Participate in quality improvement initiatives, staying updated with the latest advances in wound care and applying best practices in treatment.
--- Complete documentation accurately and in a timely manner.
--- Educate patients and their families on wound care, prevention, and management.
--- Monitor patient progress and adjust treatment plans accordingly.
Requirements:
--- Valid License as a Physician Assistant.
--- Minimum of 1 years' experience in advanced wound care is highly preferred.
--- Certification in wound care (e.g., CWOCN, CWS, WCC, WOCN) is a plus.
--- Ability to travel to multiple locations.
--- Strong interpersonal skills and the ability to work independently.
--- Excellent organizational and time management skills.
--- Wound Expert EMR knowledge preferred.
Physical/Mental Demands and Work Environment:
--- Frequent standing, walking, and sitting required for extended periods of time.
--- Requires visual and hearing acuity.
--- Able to lift a minimum of 25 pounds.
