A company is looking for a Clinical Pharmacist to manage pharmacotherapy outcomes for patients with complex chronic needs.



Key Responsibilities

Serve as a point of contact for patients, families, and healthcare providers regarding medication-related issues
Ensure accurate medication distribution, documentation, and compliance with legal and policy requirements
Oversee Pharmacy Technicians and provide timely pharmaceutical care, including medication assessments and recommendations


Required Qualifications

BS in Pharmacy; PharmD preferred
Current, active license in NJ or ability to obtain within 6 months of hire
2+ years of experience in pharmacy distribution and packaging, with exposure to geriatrics or chronic care management
Experience or strong interest in clinical pharmacogenomics preferred
