  Requirements:
  
  
  
  • NYS RN License required in hand
    
  • 5 years of healthcare experience required
    
  • Care/Case Management experience in a hospital setting
    
  • Ability to handle confidential information with discretion
    
  • Proficient in Microsoft Office (Word, Excel)
    
  • Strong communication, professionalism, and teamwork skills
    
  • Ability to assess and meet patients' age-specific needs (pediatric to geriatric)
    
    Responsibilities:
    
    
    
    • Assess patients' needs and develop appropriate care and discharge plans
      
    • Coordinate care with patients, families, physicians, and team members
      
    • Ensure care is provided in the right place, at the right time, by the right provider
      
    • Conduct readmission assessments within 30 days of discharge
      
    • Collaborate with multidisciplinary teams to implement clinical guidelines
      
    • Serve as a patient advocate and provide ongoing education
      
    • Document care processes, manage risks, and address quality concerns
      
    • Mentor new staff and act as a liaison to post-hospital care providers
      
    • Participate in care rounds to optimize resource utilization and support family education
