- Employment Type: Local ContractLarge well known social services agency looking for a full time RN to work on helping transition clients out of the hospital and back into their homes!Job Function:

- Day Shift
- Mon - Fri, 9am - 5p
- Complete medical assessments and Communicable Disease Risk Assessments within mandated time frames
- Coordinate delivery and reception of medications with pharmacy
- Track all medications received and administered as well as coordinate return of unused medications to pharmacy
- Observe self-administration of medications for detoxification according to established protocols
- Carry out urine screenings in accordance with program policy
- Provide education and consultation to individual clients and/or groups on their health needs
- Assess clients' medical record and coordinate medical care with medical director and/or outside clinics
- Assure clients' compliance with prescribe medical regimen
- Complete timely and accurate documentation on medical services rendered
- Keep supervisor informed of all relevant information regarding the residents and the program

- NYS RN license
- Knowledge of chemical dependency as a primary diagnosis.
- Assessment and case management
- Knowledge of Microsoft Office tools
- 1 year of RN experience and --working with OPWDD population. --
