ACTT consists of professionals from multiple disciplines (psychiatry, social work, nursing, rehabilitation counseling, and others) who use a team approach to meet the needs of individuals with severe, persistent mental illness in community-based settings. ACTT works according to a recovery-based philosophy and provides person-centered services addressing the breadth of an individual's needs, helping them to achieve personal goals. ACTT is generally the first-responder and single point of responsibility for meeting the client's needs, which necessitates a higher frequency and intensity of services and a low staff to client ratio. Under the direction of the ACTT psychiatrist and Team Lead, the nurse carries out a range of functions on the team, including administration of long-acting injectable medications, oversight of the medication management system, assessment and monitoring of physical health issues, and coordination with other medical professionals and pharmacy providers. This position participates in the ACT on-call rotation.The primary purpose of this position is to provide nursing functions for patients served by an assertive community treatment team. Under the direction of the team psychiatrist, the nurse will provide management and supervision of medication administration to patients in their homes and other settings.
