• Relocation Assistance (based on eligibility) -Work Performed -Clinical Services Nurse- EP Lab at Duke University Hospital -will plan and provide professional nursing care for patients in accordance with physician orders and established policies and procedures to include collecting and analyzing patient data, developing and implementing nursing care plans, establishing interpersonal relationships with patients, evaluating nursing care given and communicating to proper personnel relevant facts about patients, action taken and patient responses. Nurse clinician will enhance patient care by supporting and educating patients and staff. Facilitating patient triage and ensuring seamless coordinated care delivery by each of the complimentary components of the atrial fibrillation team. -Provide post-procedure teaching and provision of discharge instructions. Collaborate with team members in coordinating discharge planning, referral for follow up tests, return visits and treatment needs. Maintain patient record including data entry. Development and update of patient education materials per policy. Facilitate team conference meetings in relation to care of the patient or program issues as required. Follow up phone calls to assess patient status post procedure at 30 days, 6 months and 1 year. Facilitate patient triage calls when issues arise. Collaboration with the clinical service nurse in the outpatient clinic to provide continuity of care. The Nurse clinician will travel to Duke Regional and Duke Raleigh Hospital. - -Assist the Cardiovascular Short Stay staff to facilitate the recovery and discharge of patient's post Atrial Fibrillation procedure. - Provide direction and guidance in the proper use, care and control of material resources. Assist in the development and implementation of general policies and procedures.
