Description Become part of an inclusive organization with over 40,000 teammates, whose mission is to improve the health and well-being of the unique communities we serve.Summary:The University of North Carolina Hospitals and Clinics Department of Pharmacy's Acute Care Services Team has an immediate opening for a highly motivated individual interested in providing patient care to patients presenting to the emergency department (ED). -UNC Hospitals has a tradition of excellence in patient care and has been serving the residents of North Carolina for over 60 years. The University of North Carolina (UNC) Medical Center Emergency Department (ED) is a certified Level I Adult and Pediatric Trauma Center, state Burn Center, and Joint Commission Comprehensive Stroke and Chest Pain Center. Annual ED volumes exceed 70,000 and 15,000 for adult and pediatric patients respectively, with an average daily volume of 240 patients. As one of the largest referral centers in North Carolina, the department sees some of the highest acuity patients in the state, with an admit rate near 30 percent. The ED has 94 treatment spaces in 6 different areas (Adult Acute ED, Trauma, Pediatric ED, Behavioral Health, GNSH (ED Holding), and Team D (Fast Track)). -The focus of the position will be to establish consistent evening and weekend clinical pharmacy services in the emergency department setting at the UNC Hospital ED. One pharmacist will be hired to rotate weekly, working seven days in a row for 10 hour shifts followed by seven days off. The pharmacist will work in a multidisciplinary environment to provide comprehensive pharmaceutical care to both adult and pediatric patients. This position will be the pharmacist responsible for the clinical and administrative needs of patients within the emergency department on evenings and weekends. The pharmacist will work collaboratively with the current emergency department pharmacists. -The designation of Board Certified Pharmacotherapy Specialist (BCPS) and completion of a PGY1 or PGY2 residency or an equivalent level of pharmacy experience is preferred. Residency training focused in Emergency Medicine or Critical Care is also preferred for this position. Responsibilities:1. Receives prescriptions, interprets prescriptions, and reviews the patient's medication profile /history. Clarifies incomplete, inaccurate, or contraindicated prescriptions with the prescriber, nurse, or other health professional, as appropriate. Identifies and intervenes on problems related to the medication needs of the patient.2. Utilizes and analyzes patient data to provide optimal drug therapy recommendations. Assesses the patient's medication therapy regimen and makes appropriate clinical interventions with the provider. Performs protocol-driven clinical services such as IV to oral dosing, stewardship activities, etc3. Compounds and oversees the preparation and dispensing of medications including sterile products using standard formulas, processes and procedures as required.4. Complies with the medication formulary, and therapeutic substitution and other programs designed to optimize drug therapy while minimizing cost as required.5. Completes required documentation such as notations in the medical record, controlled substances documentation, cash register reconciliation, competency, etc. as required.6. Counsels patients and appropriate family members regarding pharmaceutical care issues as requested or required.7. Directs workflow of the shift and supervises pharmacy technicians, students, and/or volunteers present during that shift.8. Adheres to federal, state, local and institutional policies, procedures, standards and regulations.9. Identifies and participates in process improvement opportunities.10.
