Description Become part of an inclusive organization with over 40,000 teammates, whose mission is to improve the health and well-being of the unique communities we serve.Summary:The Medication Safety Pharmacist will play a critical role in expanding the Medication Safety Team at UNC Medical Center. This position focuses on fostering a culture of safety across the Triangle West pharmacy region, which includes the Medical Center, Hillsborough, Chatham, Youth Behavioral Health, and UNC Clinics. The Medication Safety Pharmacist is responsible for promoting safe medication practices, reducing medication errors, and enhancing patient safety throughout the organization.Responsibilities: - Collect, review, and analyze medication event reports, prioritizing improvements aimed at reducing errors and preventing patient harm. -
- Lead interdisciplinary working groups to promote a culture of safety throughout the organization. -
- Facilitate Root Cause Analysis (RCA) and Learning from Defects (LFD) processes to identify and address the root causes of medication-related incidents.
- -Oversee and manage Risk Evaluation and Mitigation Strategy (REMS) programs to ensure compliance and enhance patient safety.
- -Ensure compliance with state and federal medication safety regulations and legal requirements. -
- Provide medication safety education and training to diverse audiences, including clinical staff and leadership, to promote safe medication practices. -
- Monitor and report on infusion pump analytics, ensuring compliance with safety standards and addressing areas for improvement.
- -Stay informed on current trends in medication safety and collaborate with external safety organizations to continuously improve practices
- Provide operational, clinical, and safety recommendations related to the installation, maintenance, and enhancement of pharmacy-related systems and technologies.
- -Assist in the development, review, and implementation of policies to ensure they align with regulatory standards and support patient safety goals.
- -Actively participate in safety and quality committees to promote continuous improvement in medication safety.
- -Precept students and residents, providing mentorship and guidance in medication safety and pharmacy practice. -
- Contribute to other quality and regulatory-focused activities as assigned by leadership, supporting the organization's overall safety goals. -Other InformationOther information:Education Requirements:--- PharmD or BS Pharmacy DegreeLicensure/Certification Requirements:--- Active pharmacist licensure with the NC Board of Pharmacy or obtainable before initiation of employment. Recent graduates of ACPE-accredited colleges/schools of pharmacy may fill this role while pending initial pharmacist licensure, but are subject to limitations mandated by the NC Board of Pharmacy and must obtain licensure within 6 months of orientation or sooner if required for operations.Professional Experience Requirements:--- No prior experience required.Knowledge/Skills/and Abilities Requirements:
