A company is looking for a Clinical Pharmacist to provide medication therapy consultation and improve clinical services.



Key Responsibilities

Evaluate and document patient data, preparing reports on patient care and team progress
Conduct medication reviews and counseling appointments, and oversee other virtual pharmacist users
Develop strategies to optimize medication utilization and participate in process improvement meetings


Required Qualifications, Training, and Education

Doctor of Pharmacy (PharmD) from an accredited institution
Licensed Pharmacist in multiple states, with a California RPh license required
Completion of a PGY-1 residency or equivalent experience
In-depth knowledge of prescription and over-the-counter medications
