        Unit Specific Information
        • Case Management/Utilization Review Required: Care coordination, Plan of care
        • Assessment Required: Assess response to treatment Biopsychosocial assessment, Collect and analyze patient information Conduct initial evaluation, Develop plan of care to meet patient/caregiver needs, Development of problem list, Establish and revise goals, Mini-mental state examination, Review of Health history
        • Care Coordination Required: Assists patients with navigating the Department of Children and Families (DCF) process, Collaborative care, Coordinates with internal and external team members, Documents pertinent information to facilitate care coordination, Refers to other disciplines/resources, Supervises staff effectively and efficiently
        • Documentation/Communication Required: Discharge Summary, Documentation according to plan of care, Documentation of care delivery, plan, goals, etc, Documentation of patient/caregiver teaching/learning, Documentation reflects understanding of coverage criteria, Documents patient response
        • Counseling/Therapy Required: Facilitate discharge and contingency planning, Individual counseling, Long-range planning and decision making, Provide crisis intervention
