Job Details - Med/Surg unit specializing in post op care of orthopedic, colorectal, ENT, Oral, general, OBGYN surgeries.
Job Requirements - Primary Source Verification of professional license and/or certification, as required for the position ran within 30 days of start updated upon expiration. Attestations not allowed. Needs to be rerun every 2 years with RN license renewal & upon expiration. Temporary Licenses are NOT accepted - must have a valid permanent license.
- Proof of vaccine series or completion and approval of client medical or religious exemption form. Preferred, not required.
- Positive MMR titers ONLY from within the last 15 years. If low titer was completed within 30 days prior to start, only 1 booster is required to start and 2nd booster must be given 30 days after 1st dose. This is the only scenario that a booster(s) will be approved. After the booster series is completed, a new titer must be drawn 6 weeks after the 2nd booster to show a new positive titer.
- Completed within 1 year of start updated annually. Cannot be done on site. 3M 1860 1860S 1870+ 9205+ 6100 6200 6300 Prestige Ameritch RP88020 RP88010.
- If positive, CXR within 5 years and current Questionnaire
- Positive Varicella titers ONLY from within the last 15 years.
