A company is looking for a Post Acute Alignment Specialist who will optimize referral development for Home Health, Home Medical Equipment, and Home Infusion Therapy.



Key Responsibilities

Develop and maintain relationships with hospitals, skilled nursing facilities, and other referral sources to increase admissions
Provide education and support to referral sources while maintaining market awareness and competitive updates
Act as a liaison between referral sources and homecare services, ensuring appropriate clinical alignment and resource coordination


Required Qualifications

Degree in nursing, healthcare, or a related field
Three or more years of related sales experience in healthcare services
Strong organizational skills and ability to manage time and resources effectively
Experience in negotiation, conflict management, and customer service
