Key Responsibilities

Screen members for Diabetes Clinic eligibility and conduct detailed assessments
Provide ongoing education on diabetes management and schedule follow-up appointments
Collaborate with team members to develop care plans and optimize medication regimens


Required Qualifications

Active unrestricted Compact Registered Nurse license or Registered Dietitian license in specified states
Active Certified Diabetes Care and Education Specialist (CDCES) certification
At least 2 years of experience in clinically managing diabetes
Ability to obtain additional licenses in states where the company operates
