Key Responsibilities:

Provide customized care and treatment plans for patients in assigned Care Pods
Coordinate behavioral health care with primary care providers and monitor patient symptoms

Required Qualifications:

Master's Degree in Social Work, Counseling, or related field
Licensed to practice as an associate clinician under supervision or as an independent clinician
